Cancer Screening   Specially trained nurses do Pap smears for Cervical Cancer and breast exams.  Procedures are explained and questions answered.  Clinics are scheduled periodically and physician referrals will be made if necessary.  Please call for appointments  637-3671

Car Seat Rental    Utah law requires all children under age 2 years to ride in an approved car safety seat.  Infant and Toddler child safety seats are available at cost—or a short-term rental basis.  The fee is our purchase cost or variable for up to one-month rental .

Care-A-Van   The purpose of the program is to assist individuals in maintaining their independence and home environment in avoiding early institutional care by providing transportation to medical appointments both locally and out-of-town.  People 55 and over, people with disabilities and/or chronic disease who have no other access to their appointments are eligible.  Volunteers donate their time and vehicle in transporting to appointments.  They receive a mileage reimbursement to help cover the cost of gas, maintenance, and insurance.  Fees for this program are covered by Medicaid, donations, and a sliding fee scale based on income.

CHEC Screening for children 0-21 years of age who are eligible for a Medicaid card.  Services provided are health screening, referral and follow-up, immunizations, and case management.  Services are paid by the Medicaid Program

Case Management Changing or adding a primary care provider to a medical card.  Referrals to another physician or clinic are provided to the clients.  There is no charge for this service.

Child Health Clinic The purpose of this program is to keep children healthy and to encourage parent to enjoy their children’s development through understanding of growth patterns and to note signs of illness.  Services provided include physical examination , blood, urine, and hearing testing, development testing, comparison of height, weight, and developmental are measured and recorded.  Physician referrals are made if abnormalities are observed.  Good health practices are discussed with patients.  Consultation, guidance and referrals and given as necessary.  Immunization status is evaluated.  All children up to 21 years of age are eligible with first priority given to children under 6 years of age.  The fee is on a sliding fee scale based on income. 

Children with Special Health Care Needs  Identifies and provides services for children who are physically, mentally, developmentally, or behaviorally disabled.  The Clinic is held 3 times a year for diagnosis, evaluation, and follow-up by special physicians.  Referrals are made for other medical services (surgery, plastic repair, orthodontia, physiotherapy, and other medical aid), Orthopedic Clinic, Cardiovascular Clinic Newborns to those 21 years of age who are suspected of having handicapping conditions such as chronic disease, defect, or condition which may hinder normal growth and development are eligible.  Diagnostic visit is free.  Treatment is charged to private insurance.

Cholesterol  Education is provided on cholesterol in hopes of reducing high levels in individuals who are at risk of heart disease.  Information and education presentations to various groups, diet counseling to available for groups upon request, mini health fairs, Miners and  Power Plant Employees Presentations.  The fee is variable.  Appointments are necessary.

Communicable Diseases  The purpose of this program is rapid identification, epidemiological follow-up referral, treatment, education, and prevention.  Anyone who has or is at high risk for communicable disease is eligible.  Services include immunization clinics, epidemiological follow-up for cases, investigation of possible reportable communicable diseases, HIV testing/counseling.  The fee is determined by the services rendered.

Early Intervention (EIP) Early Intervention means services that are designed to meet the developmental needs of each individual child and the needs of the family related to enhancing the child’s development.  A multiple disciplinary approach to coordinate services for children from birth to three years of ages.  Types of services include: Assistive Technology, Audiology, Parenting Training, Health Services,  Nursing Services, Occupational Therapy, Physical Therapy, Coordination of Services, Vision Screening, Speech, Language, Transportation Reimbursement.

Environmental Health  To provide education of the population to improve sanitation and prevent diseases and enforcement of appropriate state and district standards regulation.  Concerns:  public water supplies (inspection and bacterial analysis in water lab at health department), water pollution control, radiological health, occupational Therapy, Physical Therapy,  Coordination of Services, Vision Screening, Speech, Language, Transportation Reimbursement.

Family Planning To promote responsible behavior and the well being of families and healthy babies.  To Help allow timing of pregnancies when couples are best able to care for new children and to reduce the need for abortion.  Services include: health and contraceptive education, physical exam, breast and cervical cancer screening, lab testing—HCT, urine analysis, STD, pregnancy testing, birth control, and referrals.  Uninsured or underinsured applicants are eligible.  Those under age 18 years require parental consent.  The fee is on a sliding scale dependent on income.

Health Education  The Health Education Department has many services available.  A full-time Health Educator is available to give presentations on a variety of topics.  These presentations.  There is also an information center available to the public.  Through various contracts, certain programs are provided.  Recent contracts have included Tobacco Free Utah and Injury Prevention.  Should you desire more information or a presentation for your group, please contact the Health Educator.  The fee is variable.

Home Visitation 0 - 5 Services include Home Visitation 0 - 5 years of age, Visiting nurse Consultant Services and Referral, Child Care Nurse Consultant Servies, Developmental Screening/Parent Training, Daycare Provider Training.  There is no charge for Home Visitation.  There is a charge for Daycare Provider Training.  Appointments are necessary.
